These are the three items we need to provide to APADRC upon closure of a mediation case:
- Confidentiality and Information Statement - This 2-sided sheet needs to be signed by both parties, scanned and uploaded to the Case Manager software.
- Intake Form - once the information on the form is filled out, only the front page needs to be scanned uploaded to the Case Manager software.
- A brief summary of the following. It only needs to be a few sentences typed into the Case Management software.
- What the conflict was about
- Dates of the mediation
- What resolution, if any, was reached.
